Wagner grade
A classification system for the severity of diabetic foot ulcers.
The Wagner grade classifies diabetic foot ulcers from 0 (intact skin at risk) to 5 (extensive gangrene). Medicare coverage for HBOT generally applies to Wagner grade III or higher wounds that have failed standard care.
